A school age group of children ages 5 - 6 years was out of ratio. Two caregivers were in the classroom with 38 children. The group was out of ratio by 4 children. This was corrected at inspection when the director assistant moved four children to another school age group.

This standard was evaluated and found to be out of compliance. There was not one inch of shock absorbing loose fill material or other surfacing under the slide or other playground equipment.

A staff has been determined to have pulled a child's hair while in a classroom as they were preparing to set up for naptime.

Operation conducted their own investigation by conducting interviews with persons involved, after an allegation was made against a staff member. Operation did not report the information to Child Care Regulation.

Operation failed to report an incident in which a caregiver inappropriately disciplined a child in care.

New employee was in a classroom. Required background check notification letter has not been received by operation showing the status of the new employee.

During my walk through, I observed a spray cleaing product under a sink that did not have a child safety lock. This product was removed. I also observed a mop bucket with water in a classroom where children were getting ready to nap. This was also corrected when the mop bucket was removed out of the classroom.

Operation failed to notify Licensing of a reportable illness within the required timeframe of two days.

Based on interviews it was determined that children between the ages of 12 months to 3 years old are being placed in highchairs for up to 5 or 10 minutes as a form of discipline.

In the outdoor area, there is a track made of asphalt that has two large holes, which is a safety hazard.

It was determined through the interviews that were conducted that two children were touched inappropriately by another child while be transported from school to the daycare.
